Booking and appointments

  • How do I book?By phone on +48 573 670 760, by e-mail at kontakt@hs-clinic.pl or through online booking. If you are not sure which specialist to book, call us. We will help match the visit to the symptoms and the age of the patient.
  • Does booking require a prepayment?Yes. Under our organisational rules a prepayment is required to hold a slot, paid no later than the working day after booking. It counts towards the price of the visit. It is not an extra charge.
  • How do I cancel, and do I get the prepayment back?Cancel by phone or e-mail. If you do so at least 7 days before the appointment, the prepayment is refunded. Cancelling later means the prepayment is not returned.
  • I want to move my appointment to another day. What happens to the prepayment?If you tell us by 12:00 on the day before the appointment, the prepayment moves to the new slot. Later than that, it is forfeited.
  • What happens if I am late?The specialist may see a late patient at a time of their choosing, or decline to see them. If we are the ones running more than an hour late, you may cancel the visit at no cost.

Referrals, NFZ, the first visit

  • Do I need a referral?No. HS Clinic is a private practice. You simply book a slot. No referral is needed for any of our specialities.
  • Do you accept national health insurance (NFZ)?No, all services are paid for privately and are not reimbursed by the National Health Fund. Current rates are in the price list.
  • What happens at the first visit?We start with the history: symptoms, past illnesses, medicines taken, and for children also the course of pregnancy and birth. Then the examination, a discussion of the diagnosis and the plan, and finally written recommendations. If tests or another specialist are needed, we arrange them during the visit.
  • How long does a visit last?A specialist consultation usually takes 20 to 30 minutes, a health check or newborn home visit about 30 minutes, and a physiotherapy session 45 minutes. The duration is given next to each item in the price list.
  • What should I bring?Photo ID, the child health record book, earlier test results and a list of medicines with their doses. For a vaccination, bring the vaccination record too.

Records, e-prescriptions, certificates

  • How do I get a copy of my medical records?We release a copy on request from the patient or their legal representative. After verifying identity we send it by e-mail in a password-protected file, with the password sent separately. Records can also be collected in person at reception.
  • How does an e-prescription work?We send the e-prescription code by SMS to the number given when booking. The prescription is also visible in the Polish Patient Internet Account. At the pharmacy you just give your PESEL number and the four-digit code.
  • Do you issue certificates for nursery, kindergarten or summer camp?Yes, if they follow from the examination. Mention that you need a certificate at the start of the visit so the doctor can allow time for it.
  • I lost the recommendations from my visit. Can I get them again?Yes. Call or write to reception. The note from the visit is in your medical records and we can send it the same secure way as a copy of the records.

Getting here, prams, who may bring a child

  • How do I get to you and where can I park?We are in Jeżyce, at Juliusza Słowackiego 7/8U. The nearest stops are Most Teatralny and Rondo Kaponiera. The street is in a paid parking zone (chargeable on working days), and there are private car parks nearby. Details and map: contact and directions.
  • Can I get in with a pram?Yes. The entrance is at street level, with no steps or thresholds, and a pram can be left in the waiting room. There is a baby changing table inside.
  • Who may bring a child to an appointment?A parent or legal guardian. If a grandparent or a nanny brings the child, written parental consent to the examination is required; without it the doctor may only provide emergency care.
  • Can I come with siblings or with the whole family?Yes, but each person to be examined is booked separately: one visit covers one patient. We try to put family slots back to back so you only travel once.

Payments and invoices

  • How can I pay for the visit?At reception we take card, BLIK and cash. The booking prepayment is made by bank transfer or at reception.
  • Do you issue invoices?Yes, both personal invoices and invoices to a company. Ask at reception or right after the visit and give us the billing details.
  • Can I claim the visit from my insurer?We settle visits directly with the patient. You can submit the invoice or receipt to your insurer if your policy covers private treatment; the insurer sets the terms of reimbursement.
  • When do I pay?You pay at reception before the visit, or with the doctor during or after it. The booking prepayment counts towards the price.
  • Does the price of a vaccination include the medical assessment?No. The assessment is a separate item, but you pay it only once per visit, however many vaccines are given. All rates are in the price list.
